    Risk prediction for <1 cm lateral lymph node metastasis in papillary thyroid microcarcinoma

    BackgroundBecause the diameter of the suspicious lymph nodes is less than 1 cm and adjacent to important structures in the neck, the diagnosis of small LLNM is important but difficult without the help of fine needle aspiration (FNA). There are no relevant reports of risk factors that predict the risk of suspicious &lt;1 cm LLNM.MethodsA total of 159 PTMC patients with suspicious &lt;1 cm LLNM were included in the study. Multivariate logistic regression analysis was used to identify ultrasound independent predictors of LLNM. A predictive model was developed according to multivariate logistic regression and evaluated by Hosmer-Lemeshow fit test.ResultsAge ≤ 38 years old, the largest PTMC was located in the upper part, and the presence of liquefaction or microcalcification in suspicious lymph nodes were independent risk factors for LLNM (univariate analysis P = 0.00, 0.00, 0.00; multivariate analysis P = 0.00, 0.02, 0.00. OR = 4.66 [CI: 1.78-12.21], 3.04 [CI: 1.24-7.46], 6.39 [CI: 1.85-22.00]). The predictive model for the diagnosis of suspicious &lt;1 cm lymph nodes was established as: P = ex/(1 + ex). X = -1.29 + (1.11 × whether the largest tumor is located in the upper part) + (1.54 × whether the age is ≤ 38 years) + (1.85 × whether the suspicious lymph nodes have liquefaction/microcalcification). The Hosmer-Lemeshow fit test was used to test the predicted ability, and it found that the predictive model had a good fit and prediction accuracy (X2 = 6.214, P = 0.623 &gt; 0.05). Chi squared trend analysis showed that the increase in the number of risk factors gradually increased the malignancy possibility of suspicious &lt;1 cm lymph nodes (chi squared trend test, P = 0.00).ConclusionsAge ≤ 38 years old, the largest PTMC located in the upper part, and the presence of liquefaction or microcalcification in suspicious lymph nodes were independent risk factors for suspicious &lt;1 cm LLNM in PTMC patients. Our result show that it is feasible to evaluate the malignant possibility of these lymph nodes using the number of risk factors

    Bayesian updating of soil-water character curve parameters based on the monitor data of a large-scale landslide model experiment

    It is important to determine the soil-water characteristic curve (SWCC) for analyzing landslide seepage under varying hydrodynamic conditions. However, the SWCC exhibits high uncertainty due to the variability inherent in soil. To this end, a Bayesian updating framework based on the experimental data was developed to investigate the uncertainty of the SWCC parameters in this study. The objectives of this research were to quantify the uncertainty embedded within the SWCC and determine the critical factors affecting an unsaturated soil landslide under hydrodynamic conditions. For this purpose, a large-scale landslide experiment was conducted, and the monitored water content data were collected. Steady-state seepage analysis was carried out using the finite element method (FEM) to simulate the slope behavior during water level change. In the proposed framework, the parameters of the SWCC model were treated as random variables and parameter uncertainties were evaluated using the Bayesian approach based on the Markov chain Monte Carlo (MCMC) method. Observed data from large-scale landslide experiments were used to calculate the posterior information of SWCC parameters. Then, 95% confidence intervals for the model parameters of the SWCC were derived. The results show that the Bayesian updating method is feasible for the monitoring of data of large-scale landslide model experiments. The establishment of an artificial neural network (ANN) surrogate model in the Bayesian updating process can greatly improve the efficiency of Bayesian model updating

    Activation of the hedgehog pathway in advanced prostate cancer

    BACKGROUND: The hedgehog pathway plays a critical role in the development of prostate. However, the role of the hedgehog pathway in prostate cancer is not clear. Prostate cancer is the second most prevalent cause of cancer death in American men. Therefore, identification of novel therapeutic targets for prostate cancer has significant clinical implications. RESULTS: Here we report that activation of the hedgehog pathway occurs frequently in advanced human prostate cancer. We find that high levels of hedgehog target genes, PTCH1 and hedgehog-interacting protein (HIP), are detected in over 70% of prostate tumors with Gleason scores 8–10, but in only 22% of tumors with Gleason scores 3–6. Furthermore, four available metastatic tumors all have high expression of PTCH1 and HIP. To identify the mechanism of the hedgehog signaling activation, we examine expression of Su(Fu) protein, a negative regulator of the hedgehog pathway. We find that Su(Fu) protein is undetectable in 11 of 27 PTCH1 positive tumors, two of them contain somatic loss-of-function mutations of Su(Fu). Furthermore, expression of sonic hedgehog protein is detected in majority of PTCH1 positive tumors (24 out of 27). High levels of hedgehog target genes are also detected in four prostate cancer cell lines (TSU, DU145, LN-Cap and PC3). We demonstrate that inhibition of hedgehog signaling by smoothened antagonist, cyclopamine, suppresses hedgehog signaling, down-regulates cell invasiveness and induces apoptosis. In addition, cancer cells expressing Gli1 under the CMV promoter are resistant to cyclopamine-mediated apoptosis. All these data suggest a significant role of the hedgehog pathway for cellular functions of prostate cancer cells. CONCLUSION: Our data indicate that activation of the hedgehog pathway, through loss of Su(Fu) or overexpression of sonic hedgehog, may involve tumor progression and metastases of prostate cancer. Thus, targeted inhibition of hedgehog signaling may have significant implications of prostate cancer therapeutics

    Isolating hydrogen in hexagonal boron nitride bubbles by a plasma treatment

    Atomically thin hexagonal boron nitride (h-BN) is often regarded as an elastic film that is impermeable to gases. The high stabilities in thermal and chemical properties allow h-BN to serve as a gas barrier under extreme conditions.In this work, we demonstrate the isolation of hydrogen in bubbles of h-BN via plasma treatment.Detailed characterizations reveal that the substrates do not show chemical change after treatment. The bubbles are found to withstand thermal treatment in air,even at 800 degree celsius. Scanning transmission electron microscopy investigation shows that the h-BN multilayer has a unique aligned porous stacking nature, which is essential for the character of being transparent to atomic hydrogen but impermeable to hydrogen molecules. We successfully demonstrated the extraction of hydrogen gases from gaseous compounds or mixtures containing hydrogen element. The successful production of hydrogen bubbles on h-BN flakes has potential for further application in nano/micro-electromechanical systems and hydrogen storage.Comment: 55 pages, 33figure

    Sediment resuspension is an important internal lake process in regulating nutrient cycling and ecosystem structure. Tubificid worms are widely and abundantly distributed in freshwater ecosystems and are able to alter the sediment characteristics. This study was conducted to verify the hypothesis that the alteration of sediments by tubificids may substantially influence the sediment resuspension process. Specifically, we investigated the influence of Limnodrilus hoffmeisteri (Tubificidae) on sediment resuspension using an apparatus designed to simulate the sediment resuspension process in Lake Taihu (China). We examined L. hoffmeisteri according to its density (30 000 ind.m−2) in Lake Taihu and simulated the light (3.2 m.s−1), moderate (5.1 m.s−1) and strong (8.7 m.s−1) wind processes present in Lake Taihu. Tubificids loosened the sediment through their feeding and defecation activities and increased the sediment water content. The appearance of tubificids increased the suspended solids (SS) in a 1.6 m water column under all three wind processes. During the sedimentation process, SS decreased rapidly in both the control and tubificid treatments. The total SS in the water column was significantly increased by tubificids and it changed significantly with time. In addition, the small size particles of the SS in the tubificid treatment were higher than that in the control. So, the appearance of tubificid worms (L. hoffmeisteri) enhanced sediment resuspension and raised the proportion of small size particles in SS.- Tubificid worms (imnodrilus hoffmeisteri) loosen sediment and increase the sediment water content and surface roughness. - Tubificid worms increase suspended solids in the water column during light, moderate, and strong wind processes. - Tubificid worms raise the proportion of fine particles in suspended solids
